If you have been contacted by the Department and asked to confirm you are eligible to continue to receive COVID-19 Pandemic Unemployment Payment and are still in receipt of this payment you must complete this form
If you don't complete this form by Friday 26th March 2021, your payment may be stopped.
Your MyWelfare account cannot be used to confirm eligibility for anyone other than you.
What do I need to complete this form?
Your personal details
You will be requested to provide personal details so that we can identify you.
You will be asked to declare that you continue to fulfil/meet the conditions for receipt of the Pandemic Unemployment Payment
What will happen to my current payment?
If you are still eligible for this payment then you will continue to receive your payment as normal. Once you complete this form there is nothing you need to do, unless asked otherwise by an officer.
What if I no longer meet the eligibility criteria?
If your circumstances have changed and you are no longer eligible you must close your COVID-19 Pandemic Unemployment. You can close your payment here.
